Making Grants for Hospice and End-of-Life Care and Services
Hospice Giving Foundation is an independent foundation established in 1997. HG Foundation provides leadership and direction for the long-term community health movement to help ensure exceptional end-of-life (EOL) care is available to all people, across Monterey and San Benito Counties. HG Foundation services include being the primary funder for EOL services, and providing extensive public education on planning and preparedness, understanding options for care, effective communications, as well as professional continuing education. HG Foundation works closely with funded agencies and partners to establish a more integrated model of care which benefits individuals with terminal illness, families, caregivers, and health care professionals / organizations.
Since its inception, HG Foundation has awarded more than $25 million in grants to local non-profits focused on care and services through end of life.

We approach our mission in two ways: by conducting outreach activities and educational programs to increase understanding, acceptance, and preparedness for death; and by awarding grants to qualified nonprofit agencies that deliver compassionate end-of-life care in Monterey County.

What is the organization aiming to accomplish?
To build our capacity to fund the increasing needs for care and services in Monterey and San Benito counties, California. The population is aging and the needs are increasing. The Foundation receives approximately $2 million in requests each year but have only been able to fund on average $1 million in requests each year.
To establish partnership relationships with grantees ensuring the Foundation has a strong understanding of the ongoing and/or emerging needs of the community for end of life care.
To ensure funding supports a network of services taking into account the changing needs of our community.
To build and strengthen our endowment and ensure over the long-term a consistent and stable source of funding grant requests.
What are the organization's key strategies for making this happen?
Strengthening our fundraising efforts, reaching out to new audiences, engaging existing donors more personally, to foster a deeper and longer-term relationship.
Fully engage with investment advisers to ensure that funds received are managed effectively.
Adhere to well thought out investment and spending policies to ensure adequate source of grant funds are available each year.
Careful review and evaluation of grant submissions and reports; site visits, and frequent contacts with grantees to assess progress, challenges, and/or significant accomplishments.
What are the organization's capabilities for doing this?
We have a strong staff and board committed to serving as ambassadors in the community, setting and meeting fundraising goals, and providing excellent oversight of investments so donor dollars are managed in the most effective manner possible.
Committees include individuals with subject matter expertise who expand our bandwidth for providing highest quality execution of goals.
What have they accomplished so far and what's next?
In 2014 we granted $1,085,000 to thirteen different agencies providing hospice or palliative care to adults, children, veterans, AIDS patients, and those suffering with dementia. In addition we support community education programs focusing on end of life care.
Cumulatively since our inception in 1997 the Foundation has granted over $22,000,000 dollars supporting end of life care in the community. For every one of our grant recipient organizations, our grant represents either their largest - or their only - philanthropic funding for their end-of-life programs. It is absolutely critical to their ability to deliver services.
